CORE’s OMHT model, based on research, training and experience, takes the approach that individuals released from incarceration have a greater chance of successfully transitioning back into community if they are provided pre- and post-release resources and support. OMHT is an intensive pre- and post-release clinical case management model for inmates returning to the community from Santa Rita Jail. The OMHT model is in alignment with Evidence-Based Practices (EBP) in re-entry, including continuous clinical case management pre-and post-release services conducted by trained, culturally competent therapists who navigate the jail and work with voluntary participants in OMHT. CORE Therapists who function as a Clinical Case Managers (CCMs) administer a validated risk assessment to screen all program participants for criminogenic risks and needs, which assists in developing an Individualized Re-Entry plan. The CCMs assist participants’ transition back into the community and provide referral and support services until re-entry goals are met. This assistance includes facilitating participants’ enrollment for public benefits such as Medi-Cal, General Assistance, SSI, and food stamps upon release.  CCMs are trained in Cognitive Behavioral Therapy to provide clinical interventions that address participants’ thoughts, feelings, and behavioral regulations. The CCMs also utilize Motivational Interviewing skills to mobilize the participants’ own intrinsic motivation to work towards positive change and Thinking for a Change, an intensive cognitive-behavioral curriculum, to target pro-social and proactive thoughts and decision making. 

 

OMHT relies upon a multi-disciplinary approach that establishes strong partnerships with a broad range of local stakeholders and service providers to ensure a coordination of care and to increase access to in-jail and post-release programming, including education, vocational training, restorative justice circles, parenting classes, substance abuse treatment, and faith-based services. The following are several of the partners OMHT collaborates with in providing reentry services:  Alameda County Probation Department, Behavioral Health, Health Care Services Agency, Social Services, the District Attorney’s Office, the Public Defender’s Office, and the Community Development Agency, as well as Community Based Organizations and Faith Based Organizations.
